#1 Health Class = Preferred Plus

Blood Pressure
Ages 0-60: 135/85
Ages  61+ 140/85 
No blood pressure treatment

Cholesterol
205 –  if HDL < or = to 5.0
260 If HDL < or = to 4.5

Family History:
No cardiovascular disease or cancer prior to age 60 (parents only)

Tobacco Definition
None in last 5 years

Motor Vehicle Report :
1 violation in past 3 years; 
No DUI, reckless, revocation or suspension in last 5 years


#2 Health Class = Preferred

Blood Pressure:  
Ages 0-60: 140/85
Ages 61+ 150/85  (or Preferred Plus BP reading with medication)

Cholesterol:
235 if HDL < or = to 6.0 
280 if  HDL < or = to 5.5

Family History: 
No cardiovascular or cancer death prior to age 60 (parents only)

Tobacco Definition: 
None in last 3 years

Motor Vehicle Report
1 violation in past 3 years; 
No DUI, reckless, revocation or suspension in last 5 years


#3 Health Class = Standard Plus

Blood Pressure:    
Ages 0-60: 145/88
Ages  61+: 155/88  (or Pref. Non-Tobacco BP reading w/treatment)

Cholesterol: 
250 if HDL < or = to 7.0
280 if HDL < or = to 6.5

Family History:  
One cardiovascular or cancer death prior to age 60 (parents only)

Tobacco Definition: 
None in last 1 years

Motor Vehicle Report:  
2 violation in past 3 years; 
No DUI, reckless, revocation or suspension in last 5 years


#4 Health Class = Standard

Blood Pressure:  
Ages 0-60: 145/88
Ages  61+: 155/88 (or Pref. NT BP reading w/treatment)

Cholesterol: 
250 if HDL >7.0
280 if HDL >6.5

Family History: 
One cardiovascular or cancer death prior to age 60 (parents only)
