Top 2 Health Classes

#1 Health Class = Preferred Plus

#2 Health Class =  Preferred


#1 Health Class = Preferred Plus

Blood Pressure
Up to age 69 – Treated or untreated hypertension with a 12 month average of 130/80 or lower.  Age 70 and up –  Treated or untreated hypertension with a 12 month average of 140/90 or lower

Cholesterol: 
Minimum untreated cholesterol – 125.  Maximum cholesterol – 300.   Up to age 69 – Treated or untreated readings –  – Cholesterol/HDL ration < or = 5.0.   Age 70 and up –  Treated or untreated readings – Cholesterol/HDL ration < or = 5.5.

Family History: 
Up to age 69 – No deaths of parents or siblings prior to age 65 due to cardiovascular disease.  Age 70 and up – Disregard family history of cardiovascular disease.

Tobacco: 
No tobacco or nicotine products in the past 36 months. Occasional cigar use of up to 12  cigars a year with a urinalysis negative for nicotine.

Driving Record: 
No more than 3 moving violations in the past 3 years. No DUI or license suspension in past 5 years.


#2 Health Class = Preferred

Blood Pressure
Up to age 69 – Treated or untreated hypertension with a 12 month average of 140/90 or lower.   Age 70 and up –  Treated and untreated hypertension with a 12 month average of 155/90 or lower.

Cholesterol: 
Minimum untreated cholesterol – 125.  Maximum cholesterol – 300.  Up to age 69 – Treated or untreated readings Cholesterol/HDL ration< or = to 6.0.   Age 70 and up – Treated or untreated readings Cholesterol HDL/ration < or = to 7.0.

Family History: 
Up to age 69 – No deaths of parents or sibling prior to age 60 due to cardiovascular disease.  Age 70 and up – Disregard family history of cardiovascular disease.

Tobacco: 
No tobacco or nicotine in any form in the past 24 months.  Occasional cigar use of up to 24 cigars a year with a urinalysis  negative for nicotine.

Driving Record: 
No more than 3 moving violations in the past 3 years.  No DUI or license suspension in past 5 years.
